It seems for me on a pixel variant phone (Android 8 & 10) that as well as turning off battery optimisation for the app, I also have to allow unrestricted use of data to allow the continuous pinging between phone and server with Blokada Plus: settings/apps/advanced/special access/allow unrestricted use of data for Blokada. This seems to help keep it alive.
